Applying for Eligibility:
There is no cost at any time to apply for consideration of a weight-loss grant.

The program is open to all individuals aged 18 and over as of their application date.
Applicants must be legal residents of Canada.
Applicants must commit to losing a minimum of 20 lbs, or 17% of their verified weight, whichever is greater, within the declared deadline.
Following enrollment, a Representative will contact you by telephone to learn more about your weight loss goal, and to provide you with a program orientation.
Delivery of Weight Loss Services:
You may use the weight loss company of your choice to lose your weight, provided the company you select is reasonably recognized as a credible operator in the weight loss industry. (Click to view Examples of acceptable kinds of weight loss providers)
For best results, you may combine modalities to produce a multi-disciplinary approach to your weight-loss plan of action. For example, you may receive nutrition counseling from one company, and personal training services from a different company.
You must have an arm's length relationship with the Company and/or Practitioner you select and cannot use a program or company used in the past. (For example, you cannot use a friend or family member and cannot return to a program that was used prior)
The Company and/or Practitioner you select must be disclosed to us and accepted by us before you begin treatment/services.
We make no warranties or representation regarding any services provided by any Service Provider and we are not responsible for the actions of Service Providers or the treatment they provide in their private practices or businesses.
Accredited Service Providers:
If you wish, you may elect to use the services of an 'Accredited Service Provider'.
Accredited Services Providers contribute to our funding pool and are typically health care Organizations that specialize in providing safe, effective weight loss programs that have been sanctioned by a governing body of Health Canada, and/or have been vigorously scrutinized by our Organization.
There is never any obligation to use the services of an Accredited Service Provider, however doing so may permit you to receive enhanced funding considerations such as higher co-payments of 80%.

Applicants are deemed ineligible for one or more of the reasons cited herein.
If an applicant is deemed ineligible, he or she may provide new or additional information to the Organization within 10 days of notice to re-affirm his or her application.
If the subsequent information satisfies the criteria for eligibility, the Applicant will be approved.
Applicants may not apply for funding for the purpose of satisfying prior payments made, or existing payments due, for any weight loss program that has already been completed or is currently being used.
If Applicant is currently participating in a weight loss program that is proving to be unsuccessful, applicant may stop their participation in order to apply for a weight loss grant provided the Applicant intends to re-start with a different, more suitable and effective weight loss program.
Other reasons for ineligibility may include but are not limited to:
(1) Your method of weight loss is deemed unsafe or high-risk.
(2) Your method of weight loss includes full or partial surgery.
(3) You failed to agree with the program guidelines, terms and conditions.
(4) You were attempting to defraud or abuse the program.
(5) Your application was illegible or missing important information.
(6) Your weight loss goal is uncompelling or insignificant.
(7) You did not attend for a weigh-in and/or did not remit your weight verification form.
(8) We were not able to confirm your information and/or provide you with a program orientation.
(9) We did not receive your weigh-in confirmation form.
Grants are awarded in the following manner, at the sole discretion of our Adjudication Committee:
(a) Following successful completion of the applicant's agreed upon weight loss goal, (provided the weight loss was safely achieved within the time-frame indicated for doing so, using the agreed upon method of weight loss and service provider declared by the applicant) within 45 days of the deadline from which the weight loss was to occur; and,
(b) Upon satisfactory completion, submission and verification of all program forms, requirements and documentation.
Example: If you are now 200 lbs, and declare that you want to lose 35 lbs in 18 months, then 18 months from now you will need to request the grant and demonstrate at that time that you are 165 lbs. (200lbs less 35lbs)
If you lost the 35 lbs at any time prior to 18 months, then you must still wait for the full 18 months before requesting your grant, and at the time you request your grant, you must be at your goal weight, or less than your goal weight.
Accordingly, we recommend that you do not request more time to lose your weight that you anticipate it will take for you to reach your goal. A good rule of thumb is 5lbs per month.
The date of your weigh-out must be on the date of your deadline. You may have 1 extra business day to weigh out if your deadline falls on a weekend or statutory holiday.
Once you weigh out, your documentation must be uploaded to your Funding Portal within 7 days thereafter. Accordingly book your weigh out with your physician in advance, or be prepared to attend at a walk-in clinic if your physician is unavailable.
Grant Proceeds are remitted as follows:
By cheque, in the name of the applicant, mailed by first class post to the address provided by the applicant at the time of enrollment.
WLGO reserves the right to mitigate fraud and / or abuse of its Program. Accordingly, Applicant agrees and understands that WLGO may undertake to verify documentation which in some circumstances may delay the timely remittance of Grant payments. WLGO may also request that Applicant submits to a random weigh-in audit. Failure to comply with a weigh-in audit will void Applicant's eligibility.
Applicant also consents to comply with any reasonable measure deemed necessary in order for WLGO to verify the veracity of Applicant's weight loss and or to assist WLGO in deterring fraud and abuse.
Verification and Delivery of Proceeds:
Upon request, Recipient will be required to submit the documents requested by us to verify the commencement of their weight loss program, and/or the validity of their weight loss.
Recipient will be required to provide a 'Thank You' letter.
Awards may be forfeited if the 'Thank you' letter and remaining verification documents are not signed and returned within 30 days of recipient's deadline for reaching their weight loss goal.
Recipient agrees to permit their use of the Thank You letter, name and/or grant information for promotional purposes within any medium without additional compensation to the extent permitted by law.
Due to a restricted amount of funds available each year, the Weight Loss Grants Organization may limit the number of applicants that are accepted during any given funding period and we reserve the right to defer new applicants in the event of over enrollment.
Accordingly, Applicants are urged to apply only when they are committed to losing their desired weight.
Participation is void where prohibited or restricted by law and subject to all applicable federal, provincial, state and local laws, rules, and regulations.
The program will be governed by the laws of the province of Ontario, Canada and any applicable U.S. Federal laws. Any and all legal actions or claims arising in connection with this program must be brought in the regional municipality of the City of Toronto, Ontario Canada.
We are not responsible for garbled, incomplete, misaddressed, postage-due, or misdirected applications, mail or communications, for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of application materials, loss or otherwise.
Receipt of any grant awarded is conditioned upon compliance with any and all applicable federal, state and local laws, rules and regulations. We are not responsible for electronic communications which are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in applicant’s e-mail account to receive e-mail messages. we disclaim any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with, this program, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the program should any virus, bug, technical failures, unauthorized human intervention or other causes beyond our control.
We will not send you notices to inform you of clarifications or updates to Program Terms, Conditions, or Guidelines. Accordingly, please check this page periodically for updates. The most recent update(s) are posted to the top of this page.
We reserve the right, at our sole discretion, to disqualify any applicant from this program or any other offer conducted now or in the future by us or any of our affiliates if his or her fraud or misconduct affects the integrity of the program.
By participating in this program, each applicant agrees to warrant that s/he is eligible to participate in this program.
By accepting the grant proceeds, recipient agrees to release our Organization, its directors, employees, officers, and agents, including without limitation, its advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of grant proceeds.
We are only responsible for award delivery; not responsible for award utility, quality, or otherwise. Taxes, if any, are the sole responsibility of the grant recipient.
Any attempt by an applicant to deliberately damage any website or undermine the legitimate operation of this program is a violation of criminal and civil laws. Should such an attempt be made, we reserve the right to seek damages from any such individual to the fullest extent permitted by law in any court of competent jurisdiction within Canada or the United States
